A Case of Marchiafava-Bignami Disease Involved Corpus Callosum and Cerebral Motor Cortex

Abstract
|
|
|
Background: Marchiafava-bignami disease (MBD) is a neurologic condition that presents with variable neurologic symptoms. MBD usually involves the corpus callosum and results in demyelination and necrosis. However, we report a case of MBD showed cortical involvement as well as callosal lesion.
Case Report: A 36-year-old man was found with altered mental status on a street. The past medical history could not be obtained, but he appeared to be a chronic alcoholic with significant malnutrition. On neurologic exam, he was confused, aphasic with dysarthria, and ataxic. On diffusion-weighted imaging, bilateral frontal cortical and corpus callosal lesions were found. He was diagnosed with MBD, and was started on IV thiamine therapy. After 2 months, he was discharged with improved neurologic status.
Conclusion: MBD is a slowly progressive disease that¡¯s been reported to result in high mortality. The prognosis is especially grave with cortical involvement. However, the case presented above showed favorable prognosis.
